I have recently found the nomination forms for the National Register of Historic Places have been digitized and are on NARA's website (They are also on a site from the National Parks Service, but I find the NARA citation to be more straightforward), but I'm still unsure about this citation.

Hello, Matthew. You have a document that is online at a website. You are not physically at the National Archives where you work your way through the formal organizational structure: document < file < box < series < record group. All you know about the archival hierarchy in which the document is buried is what the website is telling you. You've not verified any of it for yourself. You've only looked at the document and the website.
Therefore, all those details about the hierarchy should not be in your identification of the document itself. Those pieces of information go in the last layer where you report what the website tells you about the original document's location.
Your first layer will cite the website and, in the specific item field, the details of the document. Your second layer will be the "citing ..." layer.
Pull your copy of EE4 and go to §12.9 National Government Records: Basic Pattern ... Online Database Entries or Indexes. That example explicitly deals with materials within the National Archives Catalog database.
There's another example at §12:10 for documents imaged within the National Archives Catalog.
